Immunizations are available in the Elkhart office by appointment only. Fees are based on the vaccines given.

All appointments are scheduled through the Elkhart office at 574-523-2127.

Bring with you each visit to the clinic:

  • Complete and current vaccine record, if available.
  • Provide translator if possible.
  • If you are 10 minutes late for an appointment you may be asked to reschedule.
  • Payments are made by check, cash or credit card.
  • Fees are based on vaccines given and subject to change.
