Immunizations for Children

All appointments are scheduled through the Elkhart Office at the above number.
IMPORTANT:

Child must be accompanied by parent/guardian or an adult 18 years or older with written permission from parent.

Complete shot record showing all immunizations received since birth needs to accompany child.

Provide translator if possible.

If you are 10 minutes late for an appointment you will be rescheduled.

Payments are made by check or cash ONLY. We do NOT accept debit/credit or insurance cards.

$8.00 per vaccination. No one refused for inability to pay.
- Vaccines are given based on guidelines from the Indiana State Department of Health (ISDH) and the Centers for Disease Control and Prevention (CDC).
- Vaccines are provided by appointment at the following locations:
Elkhart Health Department - 608 Oakland Avenue
Goshen Health Department - 117 N. 2nd St.
Millersburg area:

Walk-in clinic in Elkhart (608 Oakland Avenue) on the fourth Wednesday of the month from:

October-May 1:00 p.m. to 4:00 p.m.

June - September 1:00 p.m. to 5:00 p.m.

(Doors will close early if clinic is full.)
Immunization Records
Beginning April 1, 2009 all requests for immunization records will be processed through the Elkhart Office. There will be a $5.00 charge, per client, for a certified copy of the immunization record. Additional copies, per client, requested at the same time will be $3.00 each.
